Free calcium (ionized calcium) is the active form that circulates in the blood and supports physiological functions like muscle contraction and nerve signaling. Bound calcium is attached to proteins like albumin and doesn’t directly participate in these functions. Measuring ionized calcium is more accurate in certain conditions like kidney disease or critical illnesses.
